Consumer Directed Services (CDS) Employment Forms
CDS Applicant Instructions
- Complete the four forms below.
- Come to Heartland Independent Living Center at 1010 Highway 28 West in Owensville.
*Regular business hours are 8:00 AM to 12:00 PM, and 12:30 PM to 4:00 PM Monday—Friday. - When you arrive, you’ll complete the Federal I-9 form. This form cannot be completed online.
You will be expected to provide:
- A photo ID
- Your Social Security Card
- Payment of $15.55 for the Worker Registration form, payable to Heartland Independent Living Center if you’re NOT already registered with FCSR

For In-Home, an applicant will need to complete the following forms:
Once the forms are completed online, the applicant will be contacted by HILC In-Home Care to schedule an interview.
